Donna was born January 6, 1955 to Paul and Lois Jean Bellamy in Winchester, Kentucky. She grew up in Stanton, where she graduated from Powell County High School with the class of 1973. She and John Smith were united in marriage on August 23, 1980.

Donna had placed her faith in Jesus Christ as her personal savior at an early age. She was a member of Stanton Christian Church until she moved to Greensboro. North Carolina in 2004 at which time she and John became members of Friendly Avenue Baptist Church.

In addition to her parents, she was preceded in death by an infant son, Dustin Edward Hale and a brother, Jeffery Paul Bellamy. Left to cherish her memory in addition to her husband is her twin brother, Eddie Ray Bellamy, two sons, Charles Allen Smith, his wife and daughter, Kim and Lindsay, and Jeremy Paul Smith with his wife and son, Jennifer and Tolliver. In addition to her immediate family, Donna will be missed by a host of friends, nieces, nephews, and two great-grandchildren.

She was a recipient of the Tom and T Cooper James Award for Government Improvement for her work in developing the billing process for the Guilford County Department of Public Health to obtain compensation for and tracking of potential and actual cases of lead poisoning in children. She retired as the Central Billing Unit Supervisor in 2020 after 28 years of service to both Kentucky and North Carolina. Upon retirement she began to follow her interest in decorating by opening an Etsy shop, selling floral wreaths all over the United States.  Her real passion was decorating events at church, which she enjoyed until her health declined.

A celebration of her life will be conducted on Friday, July 21st at Friendly Avenue Baptist Church in Greensboro with an hour of visitation at 3:00 PM, and a Memorial Service at 4:00 PM.  A second service will be held in Kentucky on July 29, 2023 at 11:00 AM at the Stanton Christian Church.
